How the Program Works

  1. The first step in our program is to contact Conversio Health. One of our Patient Care Coordinators will discuss the program with you in more detail. In most cases the services provided through out program will be covered by your insurance with little or no cost to you.
  2. During the initial phone call additional clinical information will be gathered regarding your condition as well as an understanding regarding any issues you may have related to your current treatment regimen
  3. The Conversio Health Complex Care Coordinators will work with your physician to provide you with the most appropriate medical supplies and/or testing regimens based upon your disease state(s) and your specific needs
  4. You will be provided education regarding the medical supplies to be used for your disease state(s), the physician's orders, as well as the proper use of the medical supplies and equipment, if applicable
  5. The Conversio Health Care Coordinators will contact you regularly to monitor your condition to ensure your care needs are being addressed. If necessary, you will receive additional follow up from Conversio Health's team of nutritionists, nurse case managers, wound/ostomy/incontinence nurses and licensed pharmacists at no charge
