We have provided this Privacy and Security Policy (“Privacy Policy”) to you to describe information collection and use practices at Conversio Health. This Privacy Policy describes the choices you can make about the way your information is collected and used.
This Privacy Policy describes how we collect, use, share, and protect information about you. By interacting with Conversio Health through our website, mobile applications, and any other Conversio Health affiliate websites where you may interact with Conversio Health, you consent to the practices described in this Privacy Policy.
Any changes in our Privacy Policy will appear on this page so that you are aware of the data we collect and how we use it. We will notify you of material changes to this Privacy Policy by posting on our websites notification that the Privacy Policy has been revised and by updating the Effective Date of the Privacy Policy.
In some circumstances, Conversio Health’s use of your information will be subject to the requirements of the Health Information Portability and Accountability Act (commonly known as “HIPAA”). For example, if you log into our mobile application and submit information regarding a prescription order, that information is subject to HIPAA’s requirements. In those circumstances, Conversio Health’s Notice of Privacy Practices and not this Privacy Policy will apply. Information We Collect
When you interact with Conversio Health or use our services, we collect certain information about you and the services you use to continually improve our products and services. An example of using our “services” would be when you visit our website or use our mobile application. We may collect and store information from you in the following situations:
Information you provide to us directly
We want you to understand how personal information you provide to us is collected and used. Personal information is any information that we can use to identify, locate, or contact you. We may collect and store your personal information when you provide it to us or to our service providers.
We receive and store information you provide to us directly, including when you:

We may combine the information collected from you when you interact with us through our websites, mobile applications, and other online services with information we receive from and about you from other online and offline sources, such as in our pharmacy, and use the combined information in accordance with this Privacy Policy or, where applicable, in accordance with the Notice of Privacy Practices.
Information we collect when you use our services.
Device Information. We may collect device-specific information when you visit our websites or use our mobile applications or services. This includes information such as hardware model, operating system, unique device identifiers, and mobile network information. We may also associate the information we collect from your different devices, which helps us provide consistent services across your devices.
Log Information. This includes details of how you used our websites or mobile applications including your IP address, clicks and page information such as the address (or URL) of the website or mobile application you came from before visiting our website or mobile application, which pages you visit on our website or mobile application, which browser you used to view our website or mobile application, and any search terms entered.
We may also collect other information from your interaction with our websites, services, and content, including computer and connection information, statistics on page views, traffic to and from the websites, and other standard weblog information.
Precise Location Information. When you use our services on your mobile phone or device and enable location services on your mobile phone or device browser, we may collect information about your physical location through satellite, cell phone tower, WiFi signal, beacons, Bluetooth, and near field communication protocols (“precise location information”). If you use our mobile application, your device may share precise location information when you enable location services for our application. Cookies and other technologies.
Cookies. “Cookies” are small data files that are sent from a website’s server and are stored on your device’s hard drive either for only the duration of your visit (“session cookies”) or for a fixed period of time (“persistent cookies”). Cookies may store user preferences and other types of information.
We use cookies to provide features and services, such as:
- Remembering your preferences and allowing you to enter your username less frequently.
- Measuring the effectiveness of our websites, services, and content.
- Providing other services and features that are available only through the use of cookies.

We may obtain or receive information from companies that we use to improve the accuracy of the information we have about you. For example, we may receive information about you from our shipping carriers, such as updated delivery and address information, which we use to correct our records and deliver your next order more easily.
How We Use Your Information

We use your personal information to provide functionality, analyze performance, fix errors, and improve the usability and effectiveness of Conversio Health’s services. We may use your information to detect, prevent, or investigate potential security breaches.
Compliance with Legal Obligations. We may use your information as necessary or appropriate under applicable law to: comply with legal process; respond to such requests from public and government authorities (including law enforcement); enforce our terms and conditions, including investigations of potential violations; detect, prevent or otherwise address fraud, security, or technical issues; protect our rights, privacy, safety, or property and; allow us to pursue available remedies to limit the damages that we may sustain. In matters involving claims of personal or public safety or in litigation where the data is pertinent, we may use personal information without your consent or court process.
Precise Location Information. When using our mobile application, we may use your precise location information to provide you with educational content and insights based on location-specific data such as the air quality index. We may use your precise location information for data analysis and research purposes.
Business Transactions. In the event that Conversio Health or some or all of our business, assets, or stock are sold or transferred (including in connection with any bankruptcy or similar proceedings) or used as security, or to the extent we engage in business negotiations with third parties, personal information may be transferred or shared with third parties as part of any such transaction or negotiation.
Business Affiliates. To the extent permitted by applicable law, we may disclose personal information to our service providers who provide services such as website hosting, data analysis, payment processing, order fulfilment, information technology and related infrastructure provision, customer service, email delivery, auditing, and other services.
Information that Does Not Identify You. We may use and disclose information that does not personally identify you (including the information described under “Cookies and Other Technologies”) for any purpose, except to the extent limited by applicable law. If we are required to treat such information as personal information under applicable law, then we may use it for all the purposes for which we use and disclose personal information. We may combine information that does not personally identify you with personal information. If we do, we will treat the combined information as personal information as long as it is combined.
How We Share Your Information
Information about our customers is an important part of our business, and we are not in the business of selling our customers’ personal information to others. We do not sell, rent, or loan your personally identifiable information. We may share your information with companies, organizations, and individuals outside of Conversio Health, only as described below.
Internally. We may share your information with our parent company or affiliates, including but not limited to, Conversio Health, LLC.
When We Work with Third Parties. We may share your information with affiliated and unaffiliated companies that perform tasks on our behalf related to our business. Such tasks include analyzing website usage data, customer service, electronic and postal mail service, processing payment, and social and other media services. These third-party service providers have access to personal information needed to perform their functions, but may not use it for other purposes.
When Sharing is Required by Law or Helps Us Protect Our Interests. We will disclose your information as necessary or appropriate under applicable law, to comply with legal process, to respond to such requests from public and government authorities, to enforce our terms and conditions, including investigations of potential violations, to detect, prevent or otherwise address fraud, security, or technical issues, to protect our rights, privacy, safety, or property, and to allow us to pursue available remedies to limit the damages that we may sustain. In matters involving claims of personal or public safety or in litigation where the data is pertinent, we may use or disclose personal information without your consent or court process.
When We Work On Business Transactions. As we continue to develop our business, we might sell or buy assets, or engage in mergers, acquisitions, or sale of company assets. Personal information may be disclosed in connection with the evaluation of or entry into such transactions or related business arrangements, or in the course of providing transition of services to another entity as permitted by law. In such transactions, customer information generally is one of the transferred business assets, but remains subject to the promises made in any pre-existing Privacy Notice, unless the customer consents otherwise. Additionally, in the event that Conversio Health or substantially all of its assets are acquired, customer information will likely be one of the transferred assets as is permissible under law.
Non-identifiable or Aggregate information with Third Parties. We may share non-identifiable or aggregate information with third parties for lawful purposes.
With Your Consent. At your direction or request, or when you otherwise consent, we may share your information.
Other than as set out above, you will receive notice when personal information about you might be shared with third parties, and you will have an opportunity to choose not to share the information.
How We Protect Your Information
We understand the importance of maintaining the security of your information, so we design our systems with your security and privacy in mind. While no data transmission or storage system can be guaranteed to be 100% secure, we use reasonable physical, technical, and administrative safeguards to protect personal information within our organization. We maintain physical, electronic, and procedural safeguards in connection with the collection, storage, and disclosure of customer personal information. Our security procedures mean that we may ask to verify your identity before we disclose personal information to you.
It is important for you to protect against unauthorized access to your passwords and to your computers, devices, and applications. We recommend using a unique password for your Conversio Health account that you do not use for other online accounts. You agree to accept responsibility for all activities that occur under your password.
What Choices Do I Have?
By interacting with Conversio Health as described herein, you consent to the practices described in this Privacy Policy. However, you do have certain choices with regard to how your information is collected and used. If you have any questions about how we collect and use your personal information, please contact our Customer Service Team at (866) 239-3784. You can also adjust settings in the Conversio Health mobile application that provide you with some options as to how your information is being used.
Email and Mobile Communications. You may choose to stop receiving email and mobile communications by changing your preferences in the mobile application, by emailing us, or by calling our Customer Service Team at (866) 239-3784. You may click Unsubscribe from the bottom of Conversio Health emails to stop email communications. You may reply STOP to our text messages to stop receiving SMS text messages. To request unsubscribing by email, contact info@conversiohealth.com.
Mobile Application. You may opt out of certain features of our mobile application. You can opt out from allowing Conversio Health to send you push notifications by adjusting the permissions in your mobile device or from the Conversio Health mobile application. You can opt out from allowing Conversio Health to access precise location data by adjusting the permissions in your mobile device. You can stop all further collection of information from the Conversio Health mobile application by uninstalling it from your mobile device. You may use the standard uninstall process available on your mobile device. Please note that if you uninstall the mobile application from your device, the Conversio Health unique identifier associated with your install and/or device might continue to be stored. If you re-install the application on the same device, Conversio Health might be able to re-associate this identifier to your previous activities.
What Information Can I Access? You can access your information, including your name, address, and demographic information in Conversio Health’s mobile application.
Can I Update My Personally Identifiable Information? You can review and update any of the information you have given us by emailing us at info@conversiohealth.com or calling (866) 239-3784. Our Customer Service Team will update your information. You can also update some information that you provided to us by logging into our mobile application. When you update information, we may need to retain certain information for recordkeeping purposes and/or to complete any transactions that you began prior to requesting such change or deletion. There may also be residual information that will remain within our databases and other records, which will not be removed.
Links to Third Party Websites. Our services may contain links to, or otherwise make available other third-party services, websites, and mobile applications that are operated and controlled by third parties. We do not take responsibility for the content or the privacy practices employed by other websites. Unless otherwise stated, any information you provide to any such third party website will be collected by that party and not by Conversio Health, and will be subject to that party’s privacy policy (if any), rather than this Privacy Policy. In such a situation, we will have no control over, and shall not be responsible for, that party’s use of the information you provide to them.
Electronic Communications Terms and Conditions
Telephone, Text, and Email Policy. By providing your residential or wireless phone number(s) and/or email(s) to Conversio Health, you expressly consent to receive marketing and non-marketing autodialed and/or prerecorded calls, text messages, and emails from or on behalf of Conversio Health at the number(s) provided. Your consent to receive texts or emails on your wireless device is not a condition of any purchase. Consent may be revoked at any time by calling us at (866) 239-3784 or emailing your opt-out request to info@conversiohealth.com with the phone number(s) and/or email address(es) you wish to opt-out. Your wireless carrier’s standard message and data rates may apply.
Text Messaging and Email Terms and Conditions. Conversio Health, or one or more of its affiliates offers access to pharmacy and healthcare service messages via recurring SMS (Short Message Service) and MMS (Multimedia Message Service) text alerts. Enrollment in text alerts requires a patient to provide his or her own mobile phone number with an area code within the 50 United States or the District of Columbia. By enrolling to receive Conversio Health text alerts, you agree to these terms and conditions, which become effective upon your enrollment.
You may be asked to verify your mobile phone number before the service will start. You acknowledge that text alerts will be sent to the mobile phone number you provide to Conversio Health. Such alerts may include limited personal information about your prescriptions, and whoever has access to the mobile phone or carrier account will also be able to see this information. Once you enroll, the frequency of text alerts we send to you will vary. You will typically receive text alerts when we have information for you about your prescriptions or other healthcare information. Conversio Health does not impose a separate charge for text alerts; however, your mobile carrier’s message and data rates may apply depending on the terms and conditions of your mobile phone contract. You are solely responsible for all message and data charges that you incur. Please contact your mobile service provider about such charges.
You may opt out of Conversio Health text alerts at any time. To stop receiving text alerts, reply STOP. Replying STOP will opt you out of any and all future Conversio Health text messages. After you submit a request to unsubscribe, you will receive one final text alert from Conversio Health confirming that you will no longer receive text alerts. No additional text alerts will be sent unless you re-activate your enrollment. You may re-activate your enrollment by replying START. The Conversio Health text alert programs are offered on an “as is” basis and: (1) may not be available in all areas at all times; and (2) may not continue to work in the event of product, software, coverage, or other service changes made by your wireless carrier. Conversio Health may change or discontinue any of its text alert programs without notice or liability to you. Conversio Health and its related companies and each of their respective officers, directors and employees are not responsible and shall not be liable for any losses or injuries of any kind resulting, directly or indirectly, from any Conversio Health text alert program or from technical failures or delays of any kind. Conversio Health reserves the right to cease delivery of text alerts to any person at anytime in its sole discretion.
